Merryfield piloting GP Connect for Nourish at Peverel Court Care

 

For Peverel Court Care, any opportunity to help advance the alignment between health and social care is one not to be missed. So when our digital care management software provider Nourish approached us asking if we would beta test their new GP Connect integration, we were delighted to help.

 

Merryfield, our nursing care home in Witney, Oxfordshire, was selected as the ideal place to beta test the new solution. By undertaking the pilot programme, Merryfield became the first care home in the country to access GP Connect via the Nourish platform.

Following a successful trial at Bartlett’s, we had rolled out Nourish to our homes as our chosen digital care plan solution in late 2020. More information about Nourish and the benefits we’ve seen can be found in our article on the digital care plan roll out.

 

GP Connect on Nourish

GP Connect is a service that allows GP practices and authorised clinical staff to share and view GP practice clinical information and data between IT systems, quickly and efficiently. The service makes patient medical information available to clinicians when and where they need it leading to improvements in the care they’re able to provide.

With the support of our digital partner Nourish, we now have access to the GP records and medical notes for our residents, facilitating more informed decision making. We will have access to clinical history, medications, allergies, history of immunisations and more. This valuable information will enhance the work we do and surface the right information at the right time.

As the first to test the GP Connect extension within Nourish, we will be shaping the way care environments interact with GP services and taking a significant step towards a joined-up care environment. We look forward to seeing how our care teams engage with GP Connect and the impact it will have on care practice.

The Care Quality Commission (CQC) have launched a new strategy which is aimed at changing the way in which care is regulated, so as to improve service delivery for users. We decided to explore how the new CQC strategy will impact the sector, and were pleased to find that at Peverel Court Care we already have projects in progress that align with all four pillars of the new strategy.

 

Whilst announcing the new strategy, the CQC have declared their ambitions for the positive impact it will have on the provision of care services across the country:

What we’ve learned from the past five years puts us in a better position for the future. Our new strategy combines this learning and experience and we’ve developed it with valuable contributions from the public, service providers and all our partners. It means our regulation will be more relevant to the way care is now delivered, more flexible to manage risk and uncertainty, and will enable us to respond in a quicker and more proportionate way as the health and care environment continues to evolve.

This new strategy strengthens our commitment to deliver our purpose: to ensure health and care services provide people with safe, effective, compassionate, high-quality care and to encourage those services to improve. Our strategy is purposefully ambitious, and to implement it we will need to work closely with others to make it a reality. We’ll review this strategy regularly so we can adapt to changes and be prepared for what the future holds.

We are acutely aware that the world is constantly evolving, and at Peverel Court Care we’re always looking to be at the forefront of innovation in social care. We have been quick to embrace technology that is able to enhance the service we deliver.

During the COVID-19 pandemic, we were able to quickly implement the use of a number of cutting edge technologies to help mitigate the risks faced by our residents and employees.

Our migration to digital care plans is another example of Peverel Court Care leading the sector to implement solutions which will, in the future, better enable the CQC to remotely monitor and care providers, allowing them to make more timely interventions when necessary. 

It is businesses like ours that blaze a trail which will eventually become ‘the norm’ and facilitate better care across the sector.

Digital Care Plan roll out

Digital Care plan roll out at Peverel Court Care - award-winning care homes in Buckinghamshire and Oxfordshire

Following a successful implementation at Bartlett’s Residential Care Home near Aylesbury in Buckinghamshire, we will be rolling out the use of digital care plans to our other homes.

 

As revealed in our Transitioning to Digital Care Plans article from October 2019, Peverel Court Care has been running a digital care plan pilot programme at Bartlett’s since the start of this year.

Our previous post detailed many of the key benefits of moving to the use of digital care plans as opposed to the paper records that have been utilised as standard in the past. However, there have also been a number of additional benefits revealed through the pilot programme.

The transition has proved to be hugely successful, both for staff and residents, and we have therefore made the decision to extend the use of the Nourish digital care plan solution to both Stone House Nursing Care Home in Aylesbury (launching September 2020) and Merryfield Nursing Care Home in Witney, near Oxford (starting October 2020).

 

Digital care plan solution

We have chosen to use Nourish as the platform for our digital care planning. Their website describes the solution as:

“Nourish enables carers to plan, record, report and co-ordinate care, all focused on the person. At a time of growing demand for care, Nourish is using technology and design to empower carers to do what they do best: help people they care for enjoy the best quality of life possible.

We give care providers the digital tools to leave the paper trail behind. Our easy to use electronic care management platform enables care teams to plan, record, report and coordinate care on the go.

It’s not about recording less information, it’s about empowering your care teams to record more accurate notes, in less time. And with our simple search and report functions, you can share critical information about the people you care for with the entire care circle.”

Our Bartlett’s pilot programme has shown sufficient evidence of the truth in these claims for us to move to roll out the solution across our other care homes in Buckinghamshire and Oxfordshire.

 

Immediate impact

Since embedding the trial at Bartlett’s, we’ve seen the global outbreak of the Covid-19 pandemic, meaning that the use of technology within the care setting has never been so critical.

Electronic care planning allows for faster reporting of symptom presentation, and for the whole care team to be made aware in a timely fashion. This has translated immediately into how care teams organise their rotas to sequence their contact with residents to minimise the likelihood of spread.

A good digital platform supports the coordination of care teams so that even in a pandemic, care is still centred around the person. GPs, district nurses and hospitals can all engage remotely with residents based on observations made available to them remotely – enabling coordination to happen between social care and health.

Electronic care planning is also reducing the reporting burden on Care Managers who are being asked to report Covid-19 symptoms, personal protective equipment (PPE) stocks, bed availability and staffing levels to several entities. Using data collection repositories enables Care Managers to report once, and for entities that are coordinating responses to consolidate this information in different ways to suit their purposes.

For our Leadership team, digital care planning allows us to review care plans remotely, providing reassured confidence; we know everything that is happening with residents and care teams. This includes which activities took place, who’s not staying as hydrated as they should or who skipped lunch, along with which carers are off. We can also learn very quickly what innovative ideas are emerging from the front line, which we can then roll out across the group; accelerating innovation in response to the challenges.

 

Resident Focus

One of the key drivers for our transition to the use of digital care plans has been our Core Value of always taking an approach based on Resident Focus. That is to say, our: “commitment to putting our residents first. We develop relationships that make a positive difference in our residents’ lives.”

In support of this, one of the pieces of feedback we received from Tudor Nanu, Deputy Manager at Bartlett’s, was:

The new Nourish system is providing us with a host of options for personalised interactions that can be used in a person-centred way to achieve an evidence-based practice. 

As soon as we started migrating care plans onto the digital platform we realised the benefit of staff being able to access information instantly. The process of auditing care notes and rolling out reports has become more time efficient using the Nourish platform.

The team quickly saw the benefits of recording care notes as they go along, as now all the documentation is at their fingertips. Staff can access risk assessments and care plans instantly, releasing valuable time for staff to spend with residents.

We are able to create more unique care plans now, which concentrate on the individual needs of our residents and can facilitate better quality in the care services we provide. In addition, the system allows us to interlink care plans and risk assessments, which is very helpful when reviewing care plans or conducting audits.

To sum up the feedback obtained from staff, they would not return to paper care plan systems.

 

Digital care plans – staff opinion

The Bartlett’s team quickly came to embrace the new technology, and Tudor continued:

Staff were very eager to learn the new system. Its use is self-explanatory and in-house Nourish champions provided one-to-one training to staff who experienced difficulties in utilising the app effectively.

Due to the fact that Nourish allows us to dictate care notes directly into the device, we are able to record more complex information about our residents. Recorded information has become easier to hand over, and the intra-mail system allows us to communicate directly with the team of any changes and updates.

As we now have the opportunity to record care notes in real-time, we can spend more quality time with our residents and can input more accurate notes on care plans as soon as we finish delivering care.

With regard to the feedback received from Bartlett’s residents and their families, Tudor added:

Residents and families welcomed the idea of having an electronic system in place that centralises the information recorded under one umbrella. It became easier to follow an evidence-based practice, as Nourish allows us to access the history of interactions recorded for our residents and we are able to review notes of our actions and the care we have provided to our clients in a more efficient way.

Resident Focus: Yvonne Bray

Following on from our recent post on the life of Bartlett’s resident Nigel Dyckhoff, we explore the globetrotting life of Stone House resident Yvonne Bray.

 

Yvonne Bray, a resident at Stone House, recently celebrated her 95th birthday. However, as we discover, she had already seen so much of the world by the time she turned 30.

Born on 23rd January 1925 in Westcliff-on-Sea, close to Southend, Essex, Yvonne was an only child. At the age of 3, she moved with her parents to Ceylon (now known as Sri Lanka), where they would remain for 8 years. Yvonne returned to England to attend boarding school at the age of 8.

World War II

In 1939, at the beginning of World War II, Yvonne was evacuated to Devon. She was aged just 14 at the time. In 1940, she travelled to India on a troopship, where she had to combine duties of keeping a lookout for German submarines with training as a shorthand typist. In 1943, to avoid the approaching Japanese army, Yvonne returned to the UK.

She spent time as a spotter on London rooftops, providing an early warning of approaching German aircraft, in the midst of barrage balloons and doodle bugs. Yvonne then worked for the Ministry of Information, before training to become a Voluntary Aid Detachment at Westminster Hospital, providing nursing care for injured military personnel.

In 1944, Yvonne met her future husband while working at the Masonic. One of her patients was Captain John Bray of the Royal Artillery, and they married 6 weeks later at St John’s Church in Surrey. Their wedding took place two weeks before D Day, on 20th May 1944, and Yvonne wore a blue dress, because there were no wedding dresses available.

Family life

After the war, John would frequently travel around the world for work. Yvonne would initially stay in England, and then go out to join him a few months later. The couple welcomed their first child in 1947, quickly followed by a second born in 1948. Both were born in Croydon, England.

In April 1949, the family travelled to Uganda by flying boat, where they remained for 5 years, and where Yvonne worked as a shorthand typist. While living in Kampala, Uganda in 1951, Yvonne and John had their youngest child. During that time, Yvonne used to nurse another baby along with her own, due to its own mother not producing enough milk.

In 1954, the family travelled to Hong Kong by boat, and they stayed there for 21 years. In later years, Yvonne ran a women’s hostel as a volunteer; helping women who arrived in Hong Kong from other countries to establish lives there.

Yvonne was a huge fan of Sir Lawrence Olivier and sent him birthday cards each year, which he acknowledged.

Retirement

Yvonne retired to Pulborough, West Sussex in 1975, and came to Stone House Nursing Care Home in September 2019. She has a very cheeky personality and is constantly having a laugh and a joke. She loves to sing and always joins in with our entertainers. She also enjoys going for country drives with her daughter Pauline, and sons Peter and Robert, and adores getting her hair and nails done.

It’s such a pleasure for all of us at Stone House to be able to care for a lady who has brought so much light to so many lives around the world. And at 95, Yvonne is still making us all smile every day.

Resident Focus: Nigel Dyckhoff

Nigel Dyckhoff - Peverel Court Care Bartlett’s Residential Care Home in Aylesbury, Buckinghamshire

 

In the first instalment of our new series exploring the amazing lives of some of our residents, we learn more about Nigel Dyckhoff; one of our Bartlett’s residents, a railway-enthusiast and retired business leader.

 

Nigel Dyckhoff is one of our residents at Peverel Court Care’s Bartlett’s Residential Care Home in Aylesbury, Buckinghamshire. However, just a few years ago, Nigel was a major player in the headhunting business. As a Partner at Spencer Stuart, one of the world’s leading global executive search and leadership consulting firms, Nigel was often asked to contribute his wisdom to publications such as Management Today, which described him as the man “who devised many of the remuneration schemes” for top earning Fund Managers and Executives at some of the World’s leading corporations.

Nigel also contributed his wealth of experience to Stephanie Jones’ 1989 book “The Headhunting Business”. He is named by the author as one of a “series of very impressive, charming, suave, witty, energetic, egotistical and even outrageous characters I met in the course of writing this book”.

With that expertise and insight, Nigel was, until fairly recently, also the Owner and Principal of the Remuneration Group, which brings together the Personnel Directors of around 30 of the UK’s Top 100 Companies. The aim of the group is to provide a forum for them to discuss and exchange information on the pay and packages of their senior staff.

 

Personal life

Born in 1935 in Manchester and raised in the city along with his sister and two brothers, Nigel earned a degree from the University of St Andrews. From there, he became a management trainee in the steel industry. Nigel then was part of the team which built and commissioned what was at that time the largest cardboard factory in Europe. He also went on to run a group of companies managing the investments in Farms and Forests on behalf of major institutional investors.

However, there’s a lot more to Nigel than just his many accomplishments in the business world. Nigel and his wife Elizabeth had three sons; Andrew, Martin and Luke.

 

Railways – a lifelong passion

Aside from family and work, Nigel’s biggest passion has always been the railway; and it is the source of his favoured hobbies. He has published several railway history books and photographic books on trains, and been a railway modeller. He has built outdoor layouts in EM gauge, O gauge and garden gauge, along with indoor layouts in OO gauge and N gauge.

In his later years, Nigel spent his time on the garden gauge layout he created at his home on the Isle of Luing in Scotland, where he lived independently until August last year. His layout with radio controlled live steam engines was considered to be a free local tourist attraction, as it could be admired by visitors over the wall from the road on the shoreline.

On Luing, Nigel was also on the committee that supervised the building of a new community centre.

For the Peverel Court Care team, it’s an honour for us to spend time with, and look after, a man who has seen, done and contributed so much during his life.

Driving Improvement through Technology

The future of Technology and Innovation in social care service provision

Examining how the use of technology in providing care can improve service delivery both now and in the future.

The delivery of adult social care is intrinsically a story of human interaction; but that’s not to say there isn’t a place for greater use of technology to aid service delivery.

While nothing can, or indeed should, replace the wonderful job that our staff do at Bartlett’s, Stone House and Merryfield; innovations in the care sector are delivering technologies that can supplement and help further improve the way we deliver services.

 

Digital Transformation in Care

For a start, we are embracing digital transformation with I.T. platforms that support integrated care. We are migrating to digital care plans, which will provide access to live information; ensuring that staff are kept up-to-date and can make informed decisions to deliver the highest quality of care. With records that are secure, accurate and compliant, evidencing care becomes more manageable, and care management more standardised.

Local Authorities can also benefit from digital care plans; being provided with continuous, easily accessible reporting delivers savings in contract monitoring. Digital platforms will allow Local Authorities to see in real-time the quality of care for each individual they are providing funding for. This will allow a smoother and faster contract monitoring process, providing a money-saving capacity and ability to provide safeguarding oversight.

Peverel Court Care believe the future lies in digital transformation and data intelligence to support better decision-making; allowing us to better predict, optimise and continue to improve care. Integrated care will be vital to both Buckinghamshire and Oxfordshire County Councils, and our digital care platform will assist in managing care between providers. However, innovation never stops and new technologies and opportunities are constantly being brought to market for the care sector.

Peverel Court Care’s Stone House nursing home in Buckinghamshire receives overall 'Good' with Outstanding in 'Responsive' in latest CQC inspection

Stone House receives overall ‘Good’ with Outstanding in ‘Responsive’ in latest CQC inspection

Peverel Court Care’s Stone House has received it’s latest glowing inspection report from the CQC, which included an ‘Outstanding’ rating in assessing our ability to be Responsive.

We were also rated as ‘Good’ in the four other assessment categories, which evaluate whether the home is considered by the inspectors to be Safe, Effective, Caring and Well-led.

The overall CQC rating awarded for our Buckinghamshire nursing home was ‘Good’ based on the most recent inspection, which was carried out on 26th February 2019.

Some of the comments from the inspector’s report included:

“Care was exceptionally person-centred and focused on ensuring people with dementia lived as independent lives as possible. The care was designed to ensure people’s maximum independence was encouraged and maintained.”

“People experienced positive outcomes regarding their health and wellbeing. People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible; the policies and systems in the service supported this.”

“Staff received good supervision and training, which provided them with the knowledge and skills to enable them to provide good care. There was a workplace culture that was open and honest. Staff told us they felt supported by the manager and they could speak with them at any time.”

While the report is indicative of a well-managed and caring environment for residents and staff alike, we’re not resting on our laurels. We will continue to strive for Stone House and the other Peverel Court Care homes in Buckinghamshire and Oxfordshire to be recognised as ‘Outstanding’ across the board in future reports. This care home is amazing. It is run by the manager and the most caring staff, many of whom have been with the company for years. The care they give their residents is second to none and the home provides them with lots of entertainment and you can see the residents enjoying these events. The meals they provide are all prepared fresh every day by the resident chef. The atmosphere when you visit the home is also something, welcoming and lots of laughter all around. A super home and I can thoroughly recommend it.

As part of our journey towards ‘Outstanding’ at Stone House, we are implementing a number of upcoming initiatives, including:

  • The introduction of bespoke training and development pathways’ for Healthcare Assistants, Registered Nurses and all other departments of the Peverel Court Care team
  • Rewards and recognition of staff for various achievements and milestones, such as high levels of attendance, attaining NVQ qualifications, referring new staff members, years of service, and the receipt of special praise via relatives
  • New and improved technology infrastructure
  • Non-clinical customer service training for all staff

We believe that by recruiting, developing, rewarding and retaining the best possible staff to deliver care to our residents within the idyllic setting of Stone House, we can continue to improve on the incredible levels of service for which we’re already being recognised.

An Incredible CQC Rating For Merryfield House Nursing Home, Witney

We are absolutely thrilled to announce that Merryfield House Nursing Home has been given a wonderful rating by the Care Quality Commission (CQC) after an unannounced inspection in October.

Unannounced inspections play a very important role in making sure the service provision that is seen is what is happening in the care home every day – not just on a day when everything has been prepared for an inspection. They are the best way to make sure that services are providing high-quality care to residents at all times.

We were visited in October 2017 by an inspector, who spoke to six residents of Merryfield and three family members. They also spoke to three relatives outside of the inspection time. They found our service to be ‘good’ across four inspection categories, finding us safe, effective, caring and well-led. We were rated ‘outstanding’ for our responsiveness.

In our report, we were praised by the inspector for showing a commitment to personalised care.

“The responsiveness of the service was outstanding. Care planning was fixed upon having the individual at the heart of their care. Relatives said support was extremely responsive because staff really understood the principles of outstanding personalised care. People were encouraged to pursue personal interests and hobbies and to access activities in the service and community. Changes in people’s needs were immediately recognised and appropriate; prompt action taken, including the involvement of external professionals where necessary. People were encouraged to raise any complaints or concerns and were given opportunities to do so.” 

At Peverel Court Care we are dedicated to providing personalised care and we are thrilled that this came across in our most recent inspection. When people come to live with us we invest a lot of time in pulling together personalised care plans to make sure that our residents have a high quality of life and are comfortable and happy.

The Importance Of Friendship At Peverel Court Care

After family, friends are usually the most important people in our lives. Modern family life can often mean our loved ones are geographically spread across the country. When we are older our children will likely have their own very busy lives to run. All of this may mean that they can’t see us as often as they would like and this is when our friendships become even more important. Friends are the family we choose for ourselves. They support us in times of need. Often just knowing that they are there for us is enough to make us feel better.

Think about some of the best times you have ever had in your life. Undoubtedly some of those times will have taken place in the presence of friends.

They’re there for us in the best and worst of times. That’s why we foster and value friendships between our residents and carers at Peverel Court Care. Here are some of the reasons we think friendships here are so incredibly important.

Friendship helps our staff do their jobs better…

The main purpose of any staff member here at Peverel Court Care is to provide high-quality care to the elderly people who live here. Friendships help our staff to do that.

“I do consider myself to be a friend to them because I feel it from my own heart.  I could not do the job that I do, which is quite solitary at times in the lounges, without having those genuine emotional feelings towards our residents.” – Peverel Court Care Carer

When you offer the kind of comprehensive care we offer, it helps greatly if you have a genuine relationship with the people you are caring for. If you feel as though the person you’re taking care of is your friend, you are much more likely to give the best care you can at all times.

Some of our residents here are uncomfortable in social situations…

Some of our residents just can’t bring themselves to join in with the group activities we offer here. Some of them just don’t want to. For these individuals, the carers visiting their rooms are the only people they see all day. Those interactions need to be friendly otherwise those people are going to feel incredibly isolated.

Some of our residents don’t have anyone else…

Our job is to make sure that those who live with us have a high quality of life. For residents without family or friends of their own left, it is important that they feel as though they have valuable and important relationships with our carers. We don’t just want them to feel cared for; we also want them to feel cared about. We can’t give our residents the high-quality care we are striving for unless people really do care about them as individuals.

It’s important for dementia and Alzheimer’s patients…

We can’t sum this up any better than one of our carers did when we asked her about genuine friendships at work.

“There have been many times when a resident has literally poured their heart out to me with real visual emotion in their expression or body language or chatted away to me ten to the dozen smiling and laughing. Sometimes I have not been able to understand what they are saying. It is so important to maintain an interest, give direct eye contact, verbalise laughter and support, handhold if appropriate and show a keen interest in what they are saying. That is friendship. That is caring. No matter how severe the level of dementia or Alzheimer’s may be, they will know if we are not responding to their needs at that time. As human beings, we would offer this support to our own friends. If we didn’t then they would not share their problems with us again. To give great care, we need to offer this level of support and friendship.”

It’s in our nature and good for our mental health….

The Mental Health Foundation says that friendship is vital for mental health.

“Friendship is a crucial element in protecting our mental health. We need to talk to our friends and we want to listen when our friends want to talk to us. Our friends can keep us grounded and can help us get things in perspective. It is worth putting effort into maintaining our friendships and making new friends. Friends form one of the foundations of our ability to cope with the problems that life throws at us.”

For the mental health of our residents, it is vitally important that we foster meaningful and fulfilling friendships with our residents.

We’re all about friendship

Being around strong, meaningful friendships makes Peverel Court Care a happy place to be. There is an atmosphere of genuine care here because our carers really do hold our residents’ dear and they want to make sure they are doing the best for them. Sometimes when we are faced with difficult situations or sadness, those friendships can make our jobs more difficult, but still, we wouldn’t have it any other way. Friendship in care is the way forward. As they say; “When it hurts to look back and you’re scared to look ahead, you can look beside you and your friends will be there.”

Technology Is Changing the World For All People Of All Ages

Quite often people in older generations complain about technology. Largely they are perturbed by the overuse of mobile phones and other communicative technologies. Part of a generation that didn’t have such luxuries, they’re concerned by the idea that people can be contacted at all times of the day and night wherever their location. So it’s ironic that this technology actually makes growing older and living away from your family much easier.

At Peverel Court Care we weave technology into our individual care plans. We’ve written before about how we give some of our residents mobile phones so that they can keep in touch with relatives, whilst for others who need additional support we arrange phone calls at a specific time so that our staff can help them connect to their family.

We’ve also written about our use of Immedicare, a system that gives us access to a digital health hub facility and experienced nurses and clinicians covering a range of specialties.

Today we wanted to tell you more about how we use Skype within our care homes to make communication with family members easier.

We’ve set up a dedicated guest network throughout all of our properties, with fibre WiFi available for all residents and their guests. This is useful for residents whose family live far away, or for those who have family or friends who are not particularly mobile themselves. Often it is most useful for those who have family living in other countries.

One of our residents Peter has one daughter who lives in Australia and another who still lives in the UK. Since we set up Skype for Peter, he speaks to his daughter using the system regularly from the other side of the world. Another of our residents, Joan, talks on Skype to her relatives regularly from Canada and Australia. Another resident speaks to her son who lives in Bournemouth on Skype when he can’t travel to visit her.

We’re always looking for new ways to enhance the technological solutions we offer at Peverel Court Care. Just recently we enquired about a piece of technology called Active Protect, which helps protect the hips of elderly individuals. We discovered this whilst researching the global technology conference CES17, looking for the latest technologies to benefit our residents. Next week we’ll be visiting the Design Museum for the New Old exhibition, which explores how designers can make life easier for the elderly.

It is our main purpose to make the lives of our residents happy, easy and comfortable. Technology is a huge part of achieving that objective. We are constantly researching new technologies. Rest assured, we’ll be at the forefront of any new developments, making sure our residents can utilise them in the best way possible.
